How to Submit a Claim
In the unfortunate event of you having to submit a claim, MiWayLife tries to make a potentially confusing process as hassle-free as possible.
Remember to keep all your documentation relating to the claim somewhere safe, as we'll most likely need it during the process.
Submit a MiFuneral Benefits Claim
Your MiFuneral Benefits include the full lump sum value assured in your policy as well as any final expenses and/or income support benefits were they selected. To submit a claim for these benefits, please fill out the claim form and attach all relevant documents. To help guide you through this process, download the “How to Submit a Claim” document.

You will need to submit the following information to claims@go.miwaylife.co.za:
1. Completed MiFuneral Claim Form
2. Certified copy of death certificate (BI5)
3. Certified copy of the South African ID document of the deceased
4. Certified copy of the South African ID document of the beneficiary/claimant
5. Notice of death(DHA1663) or notice of death by Traditional Leader(BI1680)
6. Additional in the event of an unnatural/ accidental death: Completed ‘Statement by Police’ form 7
7. Additional in the event of Still born benefit: Antenatal Records or Gestation Period Chart
Make a Claim for MiFuneral Services (repatriation and funeral arrangement services)
Services related to your policy could include the repatriation service and funeral arrangement service*. To make a claim against one of these please contact 0860111568.
*Please note that funeral arrangement costs are not covered, though beneficiaries may be eligible for discounts from select funeral service providers.
